Consent for Photography/Recording

Photography or audio or video recording of a patient, visitor, vendor, provider, or team member on Cooper premises may only take place with the signed, written consent of the person being photographed/recorded. No photographs or recordings may be taken on Cooper property without the knowledge and consent of the individuals appearing in the photo or recording.
