IRB (or IACUC) Approval Date and Approval Number*Type of Research*Basic - Basic science research refers to studies that investigate a hypothesis or question that does not have an immediate application to the diagnosis or treatment of diseases or medical conditions in humans. Typically these studies involve animals. They may involve completely de-identified commercially available cell lines from humans. If the study does not involve human subjects or involves commercially available cell lines, it does not need IRB review.Case Study - A case study/report is a detailed report of the symptoms, signs, diagnosis, treatment, and follow-up of an individual patient or small group of patients who have a similar presentation or disease state. If five or fewer patients are involved, the case report does not need IRB review.Clinical - Clinical research refers to a research study with human subjects, their specimens or their data to answer specific questions about medication, vaccines, therapies, or treatments. The project may involve generation of data through interaction with the subjects, "chart review" studies that involve collection of data from medical records or databases, or generation of data based upon examination of biological specimens. The data is either collected going forward from when the idea is generated or retrospectively, after the treatment has been completed and a question comes up which the investigator wants to answer.Nursing Research - Research done to develop evidence about issues of specific importance to the nursing profession, including nursing practice, education, administration, and informatics.Performance Improvement (PI) - Primary intent: the intent should be clear in the purpose/aim statement for the specific project. In general, performance improvement projects are aimed at improving local systems of care (non-generalizable). If the intent is to promote ‘betterment’ of a process of care, clinical outcome, etc. then the project may be considered performance improvement Performance improvement projects that will be submitted for poster consideration for the Scholars’ Forum must be discussed with the PI Review Committee prior to initiation or collection of data to insure that the project is not considered "Research".Program and Level*FellowResidentMedical StudentNurseCurrent Year of Training (med students 1-4, residents PGY1-4, fellows as PGY4-6, nursing NA)*Department*Choose Department...AnesthesiologyDiagnostic ImagingEmergency MedicineFamily and Community MedicineMedicineNeurologyNeurosurgeryObstetrics and GynecologyOrthopaedic SurgeryPathologyPediatricsPhysical Medicine and RehabilitationPsychiatryRadiation OncologySurgeryOtherIf medical student, list department of research/PI mentor.Department (Other)Division*If medical student, list division of research/PI mentor.Is/Was this project funded by any source other than Cooper or CMSRU?*YesNoIf yes, list the sponsor* Research Period: Relevant to the research performedHas this research been previously presented at Camden Scholars’ Forum?*NoYesIf yes, what year was it presented?*Choose Year...2019201820172016201520142013201220112010200920082007200620052004200320022001200019991998199719961995Has this been done entirely at Cooper?*YesNoIf no, where?*If yes, what year was it presented?
