Purpose<\/strong><\/h3>\n\n\n\n

The purpose of the Armed Forces Person of the Year Award is to honor those military members who have served or are currently serving with pride and distinction in our nation\u2019s armed forces (Army, Navy, Marine Corps, National Guard, Coast Guard, and Air Force).  We recognize the sacrifice and courage they have displayed and we appreciate their presence at Cooper.<\/p>\n\n\n\n

Criteria for Consideration<\/strong><\/h3>\n\n\n\n
    \n
  1. The Armed Forces Person of the Year award may be awarded to any Cooper University Health Care team member who has served or is currently serving in the armed forces of the United States.<\/li>\n\n\n\n
  2. Team member must have worked a minimum of one year, and not be in probationary status at the time of nomination.<\/li>\n\n\n\n
  3. Team member has no disciplinary action on file in the 12 months preceding nomination.<\/li>\n\n\n\n
  4. Team member must be in good standing with the organization.<\/li>\n\n\n\n
  5. Team member should have contributed to the community and or patient support.<\/li>\n\n\n\n
  6. Team member should exemplify Cooper\u2019s core values and standards.<\/li>\n<\/ol>\n\n\n\n

        Submission Process<\/strong><\/h3>\n\n\n\n
          \n
        1. Nomination form can be submitted online using the form below.<\/li>\n\n\n\n
        2. Nominations should be based on actions and activities consistent with Cooper Values and Strategic Imperatives. For example, support of military activity and event.<\/li>\n\n\n\n
        3. Describe why this team member should receive this award and provide specific examples.<\/li>\n\n\n\n
        4. Nominee Military Branch of Service should be included in your write up.<\/li>\n\n\n\n
        5. All nominations submitted will be reviewed by the committee.<\/li>\n<\/ol>\n\n\n\n

          Review and Selection Committee<\/strong><\/h3>\n\n\n\n
            \n
          1. The committee is multidisciplinary, consisting of nine members.\n
              \n
            • Decision will require a minimum of seven votes.<\/span><\/li>\n\n\n\n
            • There will be six alternate committee members.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n\n\n

              Selection Process<\/strong><\/h3>\n\n\n\n
                \n
              1. The committee will evaluate and select the nominations that meets the criteria.<\/li>\n\n\n\n
              2. The committee will select the Armed Forces Person of the Year based on the established criteria.<\/li>\n\n\n\n
              3. Finalist will be notified.<\/li>\n\n\n\n
              4. The Armed Forces Person of the Year will be awarded:\n
                  \n
                • A certificate (signed by our Co-Presidents\/CEOs and the department senior management).<\/span><\/li>\n\n\n\n
                • A Gold Star certificate worth 20,000 medallions to be redeemed on the Cooper Rewards Platform.<\/span><\/li>\n\n\n\n
                •  Armed Forces Person of the Year Award.<\/li>\n<\/ul>\n<\/li>\n\n\n\n
                • A recognition celebration will be held in honor of the Armed Forces Person of the Year<\/li>\n\n\n\n
                • The Armed Forces Person of the Year will be recognized on the Pulse, HR Corner, HR News, Weekly Rounds, Cooper digital boards, Pulse+, and social media channels (if they consent).<\/li>\n<\/ol>\n\n\n
